Antecedentes: el objetivo fue identificar los determinantes más importantes de la práctica para la implementación de herramientas centradas en la persona que mejoren la participación laboral de los pacientes con enfermedades crónicas. Métodos: Se realizó un estudio de método mixto consistente en entrevistas semiestructuradas, un grupo focal y una encuesta. Participaron varias partes interesadas, incluidos (representantes de) trabajadores con enfermedades crónicas, médicos de seguros, médicos ocupacionales, otros profesionales de la salud, investigadores, empleadores y legisladores. Las entrevistas semiestructuradas se realizaron para identificar los determinantes de la implementación, seguidas de un grupo focal para validar los determinantes resultantes. Para concluir, se realizó una encuesta para seleccionar los determinantes de implementación más importantes a través de la priorización por orden de importancia. La lista de verificación de implementación personalizada de enfermedades crónicas (TICD) se utilizó como marco de codificación basado en conceptos para el análisis cualitativo de las entrevistas y el grupo de enfoque. Conclusión: Este estudio identificó los determinantes más importantes desde la perspectiva de varios actores involucrados en la implementación de herramientas centradas en el cliente en salud ocupacional para trabajadores con enfermedades crónicas. Además, al priorizar los determinantes más importantes, se pueden desarrollar estrategias de implementación específicas.

 

Antecedents: l'objectiu va ser identificar els determinants més importants de la pràctica per a la implementació d'eines centrades en la persona que milloren la participació laboral dels pacients amb malalties cròniques. Mètodes: Es va realitzar un estudi de mètode mixt consistent en entrevistes semiestructurades, un grup focal i una enquesta. Van participar diverses parts interessades, inclosos (representants de) treballadors amb malalties cròniques, metges d'assegurances, metges ocupacionals, altres professionals de la salut, investigadors, ocupadors i legisladors. Les entrevistes semiestructurades es van realitzar per a identificar els determinants de la implementació, seguides d'un grup focal per a validar els determinants resultants. Per a concloure, es va realitzar una enquesta per a seleccionar els determinants d'implementació més importants a través de la priorització per ordre d'importància. La llista de verificació d'implementació personalitzada de malalties cròniques (TICD) es va utilitzar com a marc de codificació basat en conceptes per a l'anàlisi qualitativa de les entrevistes i el grup d'enfocament. Conclusió: Aquest estudi va identificar els determinants més importants des de la perspectiva de diversos actors involucrats en la implementació d'eines centrades en el client en salut ocupacional per a treballadors amb malalties cròniques. A més, en prioritzar els determinants més importants, es poden desenvolupar estratègies d'implementació específiques.

 

Background: The aim was to identify the most important determinants of practice for the implementation of person-centered tools which enhance work participation for patients with chronic health conditions. Methods: A mixed-method study was conducted consisting of semi-structured interviews, a focus group and a survey. Various stakeholders were involved including (representatives of) workers with chronic health conditions, insurance physicians, occupational physicians, other healthcare professionals, researchers, employers, and policymakers. The semi-structured interviews were performed to identify implementation determinants, followed by a focus group to validate resulting determinants. To conclude, a survey was conducted to select the most important implementation determinants through prioritization by ranking the order of importance. The Tailored Implementation of Chronic Diseases checklist (TICD) was used as concept-driven coding frame for the qualitative analysis of the interviews and focus group. The self-developed survey was based on the domains of the TICD. The survey was analyzed by frequency count of first ranking of determinants per and between domains of the TICD. Results: Various stakeholders participated (N = 27) in the interviews and focus group. The qualitative data retrieved yielded a list of determinants with additional in-depth themes according to the TICD. For the selection of the most important determinants, a survey with 101 respondents was conducted, consisting of occupational physicians, insurance physicians and workers with a chronic health condition. From the seven domains of the TICD, respondents emphasized the importance of taking into account the needs and factors associated with workers with a chronic health condition as this determinant ranked highest. Taking into account the individual needs and wishes of workers was mentioned to enable successful implementation, whereas stress of the workers was indicated to impede implementation. Other important determinants included ‘being able to work with the tools' in terms of time and usability or ‘cognitions, beliefs and attitudes of occupational and insurance physicians' to be able to use the tools. Conclusion: This study identified the most important determinants from the perspective of various stakeholders involved in the implementation of client-centered tools in occupational health for workers with chronic health conditions. Furthermore, by prioritizing the most important determinants, targeted implementation strategies can be developed.
